Home from Hospital (HFH) – Prevention & Wellbeing Worker

16 hours per week

£23,201.08 per annum pro rata

Rochdale & District Mind currently provides a Delayed Transfer of Care (DTOC) service where we support patients with Mental Health issues who are awaiting discharge from John Elliot Mental Health unit, we take referrals for patients where there is not already a statutory service provision under Section 117 aftercare – therefore we have been supporting Section 2 discharges and informal discharges.

The DTOC Coordinator is based on the John Elliot unit working across the Hollingworth and Moorside Wards, attending the ward rounds and the MDT’s liaising with ward staff and clinicians regarding appropriate referrals to our service.

The Home From Hospital Worker, will work alongside the DTOC Coordinator supporting patients who have been discharged and require further initial support in the community.  The worker will provide a link across the various mental health support systems and practical issues – such as; homelessness, housing issues, referrals for deep cleans, applications for benefits, assisting patients who have no recourse to public funds, and immigration issues.

Main duties:

  • Receive referrals, working in conjunction with our DTOC worker to offer support to patients who have been discharged from acute psychiatric hospital in patient settings – providing a continuum of support from Rochdale and District Mind after discharge with the aim of avoiding re-admission.
  • Working on supporting the patient with both practical issues and wellbeing support to ensure successful transition from the hospital setting into the community.
  • Proactively support the patients in the community to stay well and identify appropriate services to meet their needs.
  • To achieve positive outcomes by supporting personal development, building goals and aspirations and where appropriate moving onto mainstream service/activities
  • Liaising with Rochdale & District Mind Listening Lounge service in order to support service users who would benefit from a higher level of support, receiving referrals to work with and referring into the service.
  • Provide cover for our Delayed Transfer Of Care worker on in-patient units (when trained) for leave an absence.
  • To work effectively with staff from other services, elected members, health organisations, and voluntary and statutory agencies. To provide help and advice about additional services so that individuals have a full appreciation of the services available within the borough.
  • Produce a quarterly case study for commissioners in conjunction with the Delayed Transfer of Care Worker.

At Rochdale and District Mind, a Wellbeing Coordinator is there to provide ongoing support to individuals’ personal development in identifying and accessing services to meet their needs. Their aim is to achieve positive outcomes by supporting personal development, building goals and aspirations and where appropriate, moving onto mainstream service/activities.

Main duties:

  • To provide mental health & wellbeing assessments, reviews, support groups and one to one support. This role will have a specialism in one or more of the following: substance, self-harm, suicide risk, managing difficult emotions, domestic abuse, housing, debt and benefits or similar.
  • To support individuals in developing their own Wellbeing plan by identifying and accessing services to meet their needs. To achieve positive outcomes by supporting personal development, building goals and aspirations and where appropriate moving onto mainstream service/activities.
  • To implement Wellbeing based outcome tools such as the WEMWBS, ReQol 10 and Family & Friends or others as required.
  • To accurately record and maintain notes in accordance with the requirements of external funders, agencies, quality systems and policy requirements.
  • To liaise closely with other staff and volunteers to ensure a joined up, holistic approach is maintained for the individual.

You will play a key role in promoting our mission, increasing awareness of what we do and why it is important. This will include service users/families, community supporters, other Local Minds, Mind in Greater Manchester, National Mind, fundraisers, prospective funders, and Trustees.

Trustee Board Member

Voluntary Position

We are welcoming applicants for the role of Trustee. We welcome people from all the diverse communities in the area that have an interest in mental health and have a passion to help support and steer Rochdale and district Mind. We are also in particularly interested in recruiting individuals with knowledge and experience in, Finance, Law and fundraising to sit on our finance subgroup.
